Medical liaison/documentation

  • We contact the hospital, nursing home, hospice, GP, as appropriate
  • We arrange for the completion of the Death Notification Form (since 2006, every death in the State must be recorded and registered by the next-of-kin)
  • In the case of cremation we arrange for the completion of the Medical Certificate for the crematorium

  • We remove the deceased to your chosen location; hospital/nursing home mortuary, our funeral home, or a family residence.
